Clear Path/Behavioral Health Clinical Directors CQI Lymphedema Rehab Affinity Group Private Duty

One of the major benefits of belonging to the Home Care Association of New Hampshire is the networking and sharing of best practices among our members. The Association hosts quarterly Membership Meetings, where members gather for advocacy updates, conversations on current topics, briefings from guest speakers and networking luncheons. We also host the following Affinity Groups:

  • Behavioral Health– This affinity group grew out of the Clear Path program, but is open to all members of the Home Care Association of New Hampshire.
  • Home healthcare worker with senior womanClinical Directors Meetings– This group meets in Concord for briefings on clinical topics. Past topics have included:  Medicare certification survey requirements, predictive analysis by ACOs, care transition projects and Provider Orders for Life-Sustaining Treatment (POLST)
  • Continuous Quality Improvement (CQI) Forum — Home Health Quality Improvement managers gather monthly in Concord for facilitated  meetings.   The mission of the CQI Forum is to “demonstrate leadership through collaboration of member agencies to continuously identify opportunities and develop strategies to improve the quality of patient care.  The forum utilizes research, education and networking to develop resources to foster and promote continuous quality improvement.”  The Association’s innovative QI Scholars Initiative grew from the CQI Forum.
  • Lymphedema Work Group — The goal of this group is to allow for greater communication and collaboration across agencies, allowing for clinical support as well as taking advantage of potential education opportunities.
  • Rehabilitation Therapy Group — Home health rehabilitation therapy managers meet every other month in Concord for facilitated meetings on a variety of topics.
  • Private Duty Affinity GroupManagers of private duty home health agencies or departments meet regularly to discuss topics of common interest.
