Overview PRIMARY FUNCTION. The professional community health nurse is responsible and accountable for using the nursing process and making decisions that are based on the individual's education and experimental preparation. ESSENTIAL DUTIES AND RESPONSIBILITIES include, but are not limited to; • Adhere to all company policies and procedures • Demonstrate flexible and efficient time management techniques • Prioritizes workload so as to meet productivity standards and deadlines (as applicable) • Application of the nursing process by: o Assessment of clients o Nursing diagnosis o Planning of care o Implementing the care plan o Evaluating outcomes • Provide for the emotion, physical and spiritual needs of clients • Counsel and provide health teaching to clients • Provide for the safety of clients • Administer medications, to include parenteral therapy • Report Pertinent clinical assessment findings to physician • Collaborate with physician and other disciplines in case management • Develop a written client care plan and update PRN • Record client data utilizing computerized documentation systems in timely manner per Agency policy • Collaborate with Home Care Coordinators for paraprofessional client needs in the home • Involvement in on-going training of home health aides and as educational needs arise in case management • Assist with on-going Agency health status monitoring and documentation for federal and/or state regulatory requirements • Assist in coordinating Home Health Services with other community agencies • Maintain acceptable productivity per agency/industry standard through use of effective time management strategies • Accept other responsibilities in accordance with the Agency's policies and applicable laws • Act as preceptor for new RN orientation by providing field orientation experiences and related instruction when requested by Director of Clinical Services • Participates in quality assurance /utilization review activities on a monthly basis • Provides supervision of LPN in client care activities when necessary when requested by the Director of Clinical Services
Qualifications QUALIFICATIONS • Graduate of an accredited School of Nursing and current RN/LPN licensure • Graduate of NLN - Accredited nursing program. • Computer skills required. • Ability to function independently as well as collaborate with other team members. Experience in home health care preferred. • Current registered nurse license with the New Hampshire Board of Nursing required. • Must possess a valid motor vehicle operator’s license and insurance • Physical demands include occasionally lifting and moving persons or objects, as well as extended periods of sitting or standing.
