Overview Hospice Case Manager, RN “Comfort and support for end-of-life care in your home, in a nursing home, or assisted living facility. We are your local, not-for-profit Hospice for wherever you call home.” Under the direction of the Hospice Program Director, the Hospice Case Manager, RN, plans, provides, directs and evaluates nursing care in any or all-of the agency’s programs, as well as offers instructions and guidance in health practice for individuals and families. This is a full-time opportunity servicing the Keene, NH areas. Responsibilities include, but are not limited to: • Provides fair, equitable and consistent treatment of staff and delineation of responsibilities; i.e. caseload, acuity level, productivity. • Is responsible for completion of orientation of new staff in collaboration with nurse educator i.e. wound care, IV, pediatrics, Allscripts and other specialties • Schedules and facilitates patient care conferences for home care/hospice clients. • Reviews selective records including care plans and progress notes on a routine basis to assure appropriateness of care and to be sure all charting is up-to-date. • Serves as a member of the interdisciplinary team and attends patient care conferences.
Qualifications Qualifications: • Graduate of a National League for Nursing approved program, required. • Preparation in community staff nursing through a National League for Nursing approved program. • Bachelor of Science in Nursing degree, required. • Three (3) years of professional nursing experience with some supervisory level of experience. Possession of a current State of New Hampshire license to practice as a registered professional nurse, required. • Maintain current Cardiopulmonary Resuscitation (CPR) certification, required. • Have a valid New Hampshire driver’s license with unlimited access to reliable transportation, required. • Must be willing to complete an approved Hospice Volunteer Training program. • Hospice/Palliative Care and/or Oncology Certified, or demonstrate willingness to obtain certification. • IV certification and current IV Therapy experience, or ability to become IV certified.
