Overview Reporting to Homecare Program Manager, provides nursing care under RN supervision for patients in assigned clinical area. Major Responsibilities 1 Supplements and provides direct nursing care to patients and assists in the nursing process under the supervision of a registered nurse. Ensures patients have timely and accurate orders prior to delivering services. 2 Provides interventions, in accordance with the nursing plan of care, to meet patient outcomes. Documents outcomes of interventions. Assesses patient and family response to care. 3 Monitors/evaluates medical condition of patients. Documents and reports changes to appropriate personnel and team members. 4 Collaborates with other health care team members to adjust the plan of care as necessary to ensure optimal outcomes. Updates plan of care if needed under supervision of registered nurse. 5 Provides patient/family education and counseling in learning self-care techniques, in accordance with agency standards. 6 Completes timely and accurate documentation and non-visit time, transferring laptop after each visit. Completes documentation same day as visit, per agency policy. 7 Actively participates in team meetings and completes mandatory education. 8 Understands and meets expected productivity standards and obligations for weekend and holiday coverage. 9 Monitors and ensures accuracy of nursing documentation in patient medical record including medication reconciliation. Orders patient supplies in appropriate quantities and in a timely manner. 10 Maintains established agency policies and procedures, objectives, quality improvement program, safety/environment of care and infection control standards. Maintains scope of practice. Complies with professional licensure standards and code of ethics.
Qualifications Must be willing to travel; patient care services provided throughout Rockingham County; weekend (one weekend every 4 weeks) and holiday rotation Requirements · graduate of accredited LPN program · 1-3 years experience · LPN License- NH state or other Compact State · CPR certified or within 30 days of hire · Driver license – valid
