Overview Do you have a passion to assist individuals living with disabilities and other chronic conditions to live in their own homes? Do you have supervisory experience in Healthcare or the Human Services field? If so, Granite State Independent Living needs a Full Time (M-F) Long-Term Support Team Leader Service Coordinator in our Concord office for our Consumer Directed and Home Care Programs. Duties include: Supervision, training, mentoring, support and professional growth opportunities for assigned coordinator’s and staff. Organizes and manages the on-call schedule. Creates consumers care plans. Provides in-home hands on training and the development of consumer training tools/ resources, to assure all in home care workers are trained in accordance with the consumers’ needs and state regulation. Monitors and manages all in home care workers’ training and professional development. Qualifications Bachelor’s Degree and/or 2 years’ minimum experience in the healthcare or human service industry or a mix of education and experience. Supervisory experience with LPN/LNA experience preferred. Working knowledge of the needs of individuals living with disabilities and other chronic conditions. Strong marketing and customer service skills. Excellent verbal, written, organizational and interpersonal skills. Must pass a Criminal, BEAS & Motor Vehicle background check. Travel is required.
