  • Delivers home health care to clients/patients in the Maternal Child Health department whose needs range from basic to complex and are rapidly changing by utilizing the nursing process. Creates a therapeutic environment while identifying and treating human responses to actual or potential health problems through such services as case finding, health teaching, health counseling, and the provision of care supportive to or restorative of life and well-being.
KEY RESPONSIBILITIES 1.             Assesses patients' physical and psychosocial status and obtains data regarding development status and behavior to include the care of the newborn, pediatric, anti-partum, and post-partum patient. 2.     Plans and implements patient care in collaboration with the physician. 3.     Evaluates and updates appropriateness of care plan and physician orders. 4.     Communications appropriate information to patients, peers, managers, physicians, department, and other agencies involved. 5.     Integrates problem solving methodology and quality improvement in approaching problem areas related to patient care and organizational processes. 6.     Completes documentation in clinical records, time sheets and quality control logs. 7.     Orients and supervises Home Health Aides in the delivery of patient care. 8.     Ensures confidentiality of employee, legal, client/patient, budget and all company matters. 9.     Service 10.  Provides nursing care while promoting client/patient dignity and provides a calm, supportive and therapeutic environment for client/patients and their families. 11.  Acts as a resource to others (nursing students, peers, and other departments) in provision of client/patient care. 12.  Functions effectively as a member of a team. 13.  Demonstrates commitment to customer service. 14.  Positively reflects the agency within VNA, the organization and the community. 15.  Accepts and picks up work willingly, seeking other duties when work is completed. 16.  Obtains prior authorizations and documents adequately to ensure prevention of write-offs. 17.  Explains insurance coverage and cost of care appropriately to patients/significant others. 18.  Exercises care in the operation and use of equipment and reference materials. 19.  Performs routine cleaning and preventative maintenance to ensure continued functioning of equipment. 20.  Maintains work area in a clean, safe and organized manner. 21.  Participates in Total Quality Management peer review activities. 22.  Performs similar or related duties as assigned or directed.

Qualifications REQUIRED KNOWLEDGE & SKILLS 1. Ability to perform job alone following standard practices and any of many procedures. 2. Ability to transport self to and from off-site locations. 3. Adheres to NH Board of Nursing educational requirements. 9. EDUCATION/EXPERIENCE/LICENSURE 1. Education: Graduate of an accredited school of nursing required; BSN or equivalent preferred. 2. Experience: Two (2) years of experience in a maternity and/or pediatric staff nurse position in an acute care setting preferred. 3. Certification/Licensure: Current registration in State of New Hampshire as a professional nurse required. Valid driver's license required to travel throughout southern New Hampshire to provide home health care service and other job-related activities. Proof of auto mobile insurance required. Motor vehicle background report will be conducted. 4. Software/Hardware: N/A OTHER REQUIREMENTS Schedule: Work schedule may include weekend, evening, and night coverage based on VNA needs.
