Overview Physical Therapist - Home Healthcare “Skilled care for people of all ages, from newborn visits to high-tech care, to help with recovery at home. A tradition of caring in southwestern NH for over a century.” Under the direction of the Rehabilitation Manager a Physical Therapist with HCS provides and directs the provision of home physical therapy, based on agency policy and procedures, through the competent application of the therapy process. This is a full-time position. Serving Keene, NH and surrounding areas. Responsibilities include, but are not limited to: • Performs comprehensive and subjective admission and ongoing assessment of client status that includes physical, psychosocial and environmental parameters. • Formulates individualized plan of care that incorporates analysis of assessment date and current scientific findings. • Provides skilled interventions aimed at achieving realistic outcomes within a specified time-period. • Modifies and updates plan of care to reflect progress toward goals. • Implements teaching appropriate to the client's needs and within the context of his/her environment and value system. • Plans, teaches and counsels regarding physical care, measures to promote improvement/recovery, symptom/complication management, expected disease progression, social/emotional care and community resources. • Promotes client/family autonomy.
Qualifications • Graduate of a program in physical therapy approved by the Council on Medical Education of the American Medical Association in collaboration with the American Physical Therapy Association or its equivalent; • One year of professional physical therapy experience under the appropriate supervision; or; • A combination of education and experience that demonstrates the person has acquired the necessary competencies required for the position. • Possession of a current State of New Hampshire license to practice as a registered physical therapist. • Must be able and willing to learn to utilize a portable electronic field device. Must be able to manage electronic patient health information (ePHI) remote access in accordance with HCS policies and procedures. • Valid driver’s license and access to an automobile during working hours that meet the State of New Hampshire registration regulations and auto liability insurance.
