Overview Full Time and per diem positions available. POSITION SUMMARY Delivers home health care to the VNA clients/patients of all ages and families whose needs range from basic to complex. Provides direct and indirect skilled nursing care to VNA patients. Creates a therapeutic environment while identifying and treating human responses to actual or potential health problems through such services as case finding, health teaching, health counseling, and the provision of care supportive to or restorative of life and well-being. KEY RESPONSIBILITIES 1. Plans daily work activities in conjunction with Scheduling Department, Manager and the Clinical Leader based on program needs. 2. Assesses patient and interviews family to obtain a comprehensive clinical and environmental assessment of the patient. 3. Plans and implements patient care based on patient needs and assessment in collaboration with the physician and revises plan according to patient need/physician orders. 4. Responds appropriately to emergency patients needs. 5. Monitors patient response to treatment and communicates appropriate information to patients, peers, managers, physicians, department, and other agencies involved. 6. Integrates problem solving methodology and quality improvement in approaching problem areas related to patient care organizational processes. 7. Completes documentation in clinical records, time sheets, and quality control logs. 8. Orients and supervises Home Health Aides in the delivery of patient care. 9. Adheres to VNA safety, organizational, and ethical standards. 10. Adheres to VNA policies, protocols and procedures, including infection control standards. 11. Ensures confidentiality of employee, legal, client/patient, budget and all company matters. 12. Provides nursing care while promoting client/patient dignity and provides a calm, supportive and therapeutic environment for client/patients and their families. 13. Acts as a resource to others (nursing students, peers, and other departments) in provision of patient/client care. 14. Provides technical and clinical expertise when questions arise from patients, staff, case managers or referral sources. 15. Functions effectively as a member of a team. 16. Demonstrates commitment to customer service. 17. Positively reflects the agency within VNA, the organization and the community.
Qualifications EDUCATION/EXPERIENCE/LICENSURE 1. Education: Graduate of an accredited school of nursing required; BSN or equivalent preferred. 2. Experience: Equivalent of one (1) year in a staff nurse position in acute care setting preferred. 3. Certification/Licensure: Current Licensure in State of New Hampshire as a professional nurse required. Midline Certification preferred. BLS/CPR Certification required.
