The Alliance is co-hosting “Communicating with Patients & Caregivers”. This is part of continuing collaboration with hospitals, SNFs, and the QIN-QIO to improve Transitions of Care. It will be held from 9:00am to noon at the NH Audubon Center in Concord. We encourage you to send members of your team (limit of 3). This is a free meeting, but registration is required.
