CMS Proposes a F2F template for Physician EMRs

This week, CMS conducted a Special Open Door Forum that introduced participants to a suggested Electronic Clinical Template for the home health face to face encounter documentation. The initiative is from of the Office of the National Coordinator for Health Information Technology (ONC,) Standards & Interoperability (S&I) Framework. The S&I Framework is responsible for developing standard electronic data sets for electronic medical records.

The template is not intended to be mandatory for physicians, but is a data set that when completed will provide information required to satisfy the home health F2F encounter requirements. In addition, the completed evaluation will be a part of the medical record.   A power point presentation, which described an overview of the project, was the focus of the open door forum with no discussion of the contents contained within the actual template itself.   The template is several pages long with many questions that require a description of the patient’s condition. Click here to view the power point presentation and the suggested clinical template.

CMS will be conducting webinars and teleconference calls on the initiative every Wednesday from 1:00pm – 2:00pm ET. The calls are open to the public and can be accessed hereIn addition, CMS will be hosting a series of Special Open Door Forum calls to provide an opportunity for physicians, home health agencies and/or all other interested parties to provide feedback on the suggested Electronic Clinical Template.

All Open Door Forums are from 3 PM to 4 PM EST on the following dates:Thursday, May 8th; Thursday, June 19th; and Wednesday, July 16th
