Nashua, NH- Souhegan Nursing Association (dba St. Joseph Home Health Care) announced this week that it will transition its services to Home Health & Hospice Care in Merrimack (HHHC). St. Joseph Home Health Care is a home health care subsidiary of St. Joseph Healthcare in Nashua
“This decision was not made lightly, “ says Dr. Richard Boehler, President of St. Joseph Hospital. “Since last fall it has become increasingly clear that a unified approach to home health care will best serve our patients and their families in this community. “
“The medical reimbursement climate as well as a declining patient census have all combined to make this move,” he says. “ We are very fortunate as Home Health & Hospice Care gives the same exceptional service that we have always offered our patients.”
St. Joseph Home Health Care will cease operations on June 30, 2015. All patients will have a choice of moving their services to Home Health & Hospice Care or other providers. Both agencies are working together to ensure a smooth changeover.

St. Joseph Home Health Care was acquired in 1996. Based in Milford, it provides nursing and home health aide services as well as physical and occupational therapy and medical social work services to over 1100 patients in 15 communities. The organization’s hospice program was closed in December 2014.
All 50 employees of St. Joseph Home Health Care will be considered for employment at Home Health & Hospice Care to handle the additional work load.
Home Health & Hospice Care of Merrimack serves over 25 communities in the greater Nashua area. It offers services including traditional home care, palliative care, hospice services, rehabilitation therapies, medical social work, foot care clinics and bereavement groups. In 2000 the organization opened the ten suite Community Hospice House in Merrimack.
