Group Focused on Stroke Care Releases Updated Material

Recently the New Hampshire Stroke Collaborative released two revised documents for patients and their families,  The Stroke Survival Guide and a resource lists.  The New Hampshire Stroke Collaborative is a member of the North East Stroke Consortium, which has focused on 7 key components of the Stroke System of Care Model (SSCM1):

“Working together to prevent strokes, improve outcomes, and inspire hope.”

  1. primordial and primary prevention;
  2. community education;
  3. notification and response of emergency medical services (EMS);
  4. acute stroke treatment;
  5. sub acute stroke treatment and secondary prevention;
  6. rehabilitation; and
  7. continuous quality improvement activities.
    The North East Cerebrovascular Consortium (NECC) was established as an independent organization in 2006 to improve stroke care in an 8-state region (Maine, New Hampshire, Vermont, Massachusetts, Rhode Island, Connecticut, New York, and New Jersey). The NECC improves care and reduces disparities across the northeast region through collaboration, coordination and dissemination of best practices within the stroke systems of care.
