Member Benefit Spotlight: Affinity Groups

One of the major benefits of belonging to the Home Care Association of New Hampshire is the networking and sharing of best practices among our members through Affinity Groups.  All our groups are member-driven; participants decide the topics and agendas. Behavioral Health Managers Group —  This affinity group grew out of the Clear Path Project, but is open to all HCANH members.  The group invites in speakers on behavioral health topics and shares case studies. The group meets quarterly.

Clinical Directors Meetings— This group meets every other month in Concord for briefings on clinical topics. Past topics have included:  Medicare certification survey requirements, predictive analysis by ACOs, care transition projects and Provider Orders for Life-Sustaining Treatment (POLST).

Continuous Quality Improvement (CQI) Forum – Home Health Quality Improvement managers gather monthly in Concord for meetings facilitated by Sherry Ovens- Burleigh from Rockingham VNA. The mission of the CQI Forum is to “demonstrate leadership through collaboration of member agencies to continuously identify opportunities and develop strategies to improve the quality of patient care. The forum utilizes research, education and networking to develop resources to foster and promote continuous quality improvement.”

Rehabilitation Therapy Group – Home health rehabilitation therapy managers meet quarterly in Concord for meetings on regulatory and practice topics.. The group is facilitated by Amy McWeeney, PT. from Concord Regional VNA.

Private Duty Affinity Group – Private duty managers meet regularly to discuss topics of common interest. This relatively new group is facilitated by Rebecca Hutchinson, the former director of In-Home Care for the Ascentria Care Alliance.
