The NH Hospice and Palliative Care Organization (NHHPCO), and the Home Care Association of NH (HCANH) are now operating as the Home Care, Hospice and Palliative Care Alliance of NH to better address patient needs across the care continuum. Please join us in building new relationships, nurturing existing ones, and exploring new opportunities. This conference will provide opportunities to network with statewide colleagues while you promote what you do best.
We welcome all relevant community partners from healthcare, legal services, funeral services, bereavement, integrative therapies, jewelry, scarves, etc. and our popular bookstore The diversity of our exhibitors adds to the interest and fun for attendees. With up to 200 participants, we receive high marks for quality of the presentations and facility at this annual event.
The conference theme; Keeping the Patient Voice Alive: The Best Care Possible at the End-of-Life, emphasizes the need to ask the right questions in advanced care planning discussions and listen well to honor the individual wishes of each person living with progressive illness. Together we make a difference in improving care for those in our collective care.
