Foundation for Healthy Communities Facilitator Training
The Foundation for Healthy Communities is offering two programs in April to train health care professionals whose work includes discussions with patients about Advance Directives, including Provider Orders for Life-Sustaining Treatments (POLST.)
- Advance Care Planning Facilitator Training, April 5th, 8:30am – 4:00pm
- POLST Facilitator Education Program Certification Workshop, April 12, 8:30am – 4:30pm
Both events will be held at NHHA, 125 Airport, Concord. The cost is $45 per session. The Foundation for Healthy Communities is the state’s leading resource on advance care and end of life care planning. The National POLST Paradigm Task Force recently endorsed the Foundation as the state POLST Program, a designation that is given to only one entity per state.
