Clinical Documentation Improvement

Including Virtual Visits

Wednesday, December 16, 2020

1:00pm-2:30pm

Program Description:

Clinical Documentation is “telling the patient’s story” and clearly showing the value of the home care services. Few people see the patient and everyone else must read the story. To provide a foundation for CDI Programs, agencies must educate clinicians and provide continuous analytic feedback to staff and management. Documentation does not need to be excessive, but it must be accurate and defensible.

Objectives:

  1. Be able to define CDI
  2. Gain knowledge of documentation to meet regulatory requirements
  3. Monitory CDI Programs with Benchmarks and Metrics

Content Outline (Outline the steps to achieve each objective mentioned above)

  2. Accurate data collection at the point of care
  3. Objectives of data collection for documentation
  4. Documentation completeness and quality to include virtual visits
