Excellent documentation is as crucial to an agency’s success as is excellent care.   It supports payment, confirms compliance with regulations, and provides a legal defense against claims of malpractice.   The purpose of this presentation is to arm agency leaders with information and strategies they can bring back to their clinicians to help them “get” and “deliver” good documentation.   The presentation includes a presentation-within-the-presentation, designed to be delivered to an agency’s clinicians.

Program Objectives:

  • Describe the care planning process as a framework for enhancing documentation.
  • Discuss specific strategies for achieving documentation that supports coverage, is compliant with regulation, and is legally defensible.
  • Deliver an in-service at your agency that enhances clinical documentation.

 

Faculty:  Mary Narayan, MSN, RN, HHCNS-BC, COS-C, is a Clinical Nurse Specialist, with Narayan Associates in Vienna, VA.  Mary provides home health clinical

nurse specialist services in education and quality improvement activities, including integrating documentation in the care planning process.  She served on the

Taskforce to review the American Nurses Association’s Home Healthcare Nursing: Scope and Standards of Practice and serves as a legal nurse consultant.
