The Patient Protection and Affordable Care Act has prompted an environment that is encouraging non-traditional collaboration across the care continuum. In response to the need to better manage the health of a population beyond the confines of a hospital setting, healthcare providers and payers have begun to integrate the social sector into their strategic partnerships. In addition, through the implementation of initiatives such as Bundled Payments for Care Improvement, Accountable Care Organizations, Medicaid Managed Care, and Value-Based Purchasing, the impact of social determinants on health outcomes is becoming more widely recognized and accepted. As such, healthcare organizations are being held accountable for outcomes that reach beyond their own infrastructure and expertise. Rather then building a new service from within, healthcare entities are recognizing the value of community partners in supporting the aim of optimal performance across the continuum.
