  • Dr. Susan Adams, NAHC Board member, and Vice President of Alliance Integration at Masonicare, Wallingford, CT.  Susan will be discussing Masonicare’s experience with CMS’s Model 3 bundled payment demo, along with opportunities for home care agencies to partner with skilled nursing facilities.
  • About Dr. Susan Adams
    Dr. Adams began her career at Yale New Haven Hospital as a Pediatric nurse. After many years of caring for the smallest and the frailest she added Home Healthcare to her experience and began caring for all ages of those in need. A love of learning took her from her undergraduate BSN from UCONN to an MHSA from St. Joseph’s College and on to a PhD from LaSalle University. She has worked and continued her learning in a variety of settings both in the not for profit community and the for-profit world. She currently serves on the CT Healthcare at Home Board of Directors as well as having been elected to the National Association for Home Care’s Board of Directors as the Region 1 Director, representing the New England states. She was appointed to the State of CT SIM project serving on the Practice Transformation Task Force and has been selected to as a member of the TEP (Technical Expert Panel) for a CMS project to develop a cross-setting discharge to community measure to meet the mandate of the improving Post-Acute Care Transformation Act of 2014 (Impact Act). In her current role as Vice President for Alliance Integration for Masonicare she is engaged in crossing the continuum of Masonicare and reaching out to other providers to work together to meet patient’s needs wherever they are and increase their satisfaction and outcomes while striving to reduce cost.
  • John Poirier, CEO of the New Hampshire Health Care Association, will also join us to discuss challenges and opportunities facing SNF’s in New Hampshire.
