Position Description

The registered nurse’s responsibility includes collecting client health data and working with the patient/family to set attainable goals. Registered nurse must have strong assessment, triage, IV, and wound care skills. The nurse must be able to work closely with weekend supervisors to perform visits as assigned. Job Description:

Assists in the coordination of home health services by working with the clinical management or coordinators for all departments, clinical staff, provider offices, hospitals and all other referral sources. Updates liaisons with hospitalizations of patients as needed. Hospice Community On Call Registered Nurse – $5,000 sign-on bonus!

Current Work Schedule: Friday, Saturday & Sunday 3 p.m. – 8 a.m. Patient visits from 3 p.m. – 5 p.m., on-call hours from 5 p.m. – 8 a.m. Part time, salaried. Potential for full time, salaried by providing patient visits in the community two days per week.

The Hospice Community On-call Registered Nurse provides on-call coverage for the agency’s hospice program during the hours that the agency is closed and completes hospice visits. Upon request of the Administrator on Call will assist the other programs.

Essential Functions

Responds to and/or triage calls during on-call hours in a timely manner and documents all telephone contacts, patient care observations and interventions.

After triaging calls, document outcomes of call along with patient/caregivers responses and if any further instruction is needed in Home Care System.

Prioritizes calls and completes visit for patients as needed. Communicates with and supports after-hours staff in assessing and treating patient’s needs.

Documents all interim verbal orders obtained from the physician accurately in Home Care System and communicate to all members of the team to ensure implementation.

Receives and records accurate and thorough referral information requests for services. Notifies the Intake Nursing Staff of the referral.

Records the outcome of all inquiries and referrals received and provides appropriate follow-up when an inquiry or initial referral cannot be serviced during on-call hours.

Provides skilled professional nursing care to patient/family/caregivers as prescribed by the physician and in compliance with agency policies when called upon after-hours.

Completes clinical documentation of all after-hours calls and visits, including problems and interventions within the Home Care System timely and in accordance with agency policies and procedures.

Completes an on-call log on telephone calls as received. Submits on a daily basis.

Communicates with the interdisciplinary team to ensure proper patient follow-up on previous night’s after-hours calls.

Provides accurate information to customer making telephone inquiries regarding Home Care. Hospice, Personal Home Services and other agency services.

Notifies after-hours administrator on-call when necessary and/or when guidance needed.

Communicates clearly with professional staff regarding expected “call-backs” and relays information to physicians and others.

Maintains a positive working relationship with referral sources and individuals seeking information.

Other Job Functions

Demonstrates flexibility and accepts other duties as assigned

Demonstrates knowledge of Medicare Regulations and delivers care in accordance with Medicare Condition of Participation (COP) for hospice.

Arranges to have needed supplies and equipment in the patient’s home. Appropriately uses supplies and equipment. Seeks out education for any unfamiliar procedures.

Granite VNA is hiring a Weekend Clinical Resource Nurse! 16 hours per week, full-time with benefits. This position requires the nurse to work in-house. This is not a remote opportunity.

The Clinical Resource Nurse helps achieve our mission by providing skilled and palliative care assessment when telephone triaging calls from home care and hospice community patients and families, skilled and assisted living facilities, John H Whitaker Place, physicians and their office staff, pharmacies, durable medical equipment providers and other type of calls related to active patients. Some essential job functions include:

Clinically triage phone calls from patients/families/caregiver by providing skilled and palliative care assessment, problem solving and instruction along with assisting to coordinate home visits as indicated for patients.
Clinically coordinate the scheduling of complex IV, Medication Reconciliation and Wound Care referrals.
Provides guidance including pain management and symptom control; conducts assessments and evaluations and provides education and support care to patients and caregivers in collaboration with the nurse case manager.
Coordinate patient care with the physician office and enter orders as indicated. Communicate outcomes with clinical team.

Completes comprehensive documentation about the calls received/initiated in the patient’s medical record using the SBAR guide.

Supports Scheduling with determining priority of assigning open visits.
Assist with chart review and monitoring of Quality Improvement and Patient Safety activities as requested.
Reviews orders received by intake that impact the patient’s plan of care and makes appropriate changes.
Acts as a resource to the Home Health and Hospice clinical team.
Demonstrates flexibility in the role by accepting other duties as assigned for which qualified.

JOB SUMMARY:

Delivers care to patients and families with needs ranging from basic to complex utilizing the nursing process. Creates a therapeutic and caring environment for patient and families. ESSENTIAL FUNCTIONS AND RESPONSIBILITIES

Interview the client and or family to obtain a comprehensive history and to obtain data about client/patient’s developmental status and behavior.
Examine the client to obtain objective physical data.
Review available clinical data from referral and lab results, etc. as part of database.
Complete a physical and psychosocial assessment based on interview, examination and review of available clinical data to complete Oasis or Initial visit.
Document data and assessment on appropriate form according to guidelines.
Assess client’s need for Palliative Care and refers appropriately.
Reassess client status at established time frames and as indicated by the client status. Reports status and changes in status as appropriate.
Assess potential for development of crisis situations based on combination of subjective and objective cues.

Evening Care Registered Nurse (RN)

We are looking for a registered nurse interested in an joining our Extended Care Team as an Evening Care Registered Nurse responsible for completing scheduled and PRN visits. This position works closely with the Overnight On-Call Nurse to ensure all patient needs are met and excellence in care is provided. The Evening Care Registered Nurse will make home visits as needed to meet the needs of Homecare patients, as well as cross train to provide back up support to the on-call nurse for Hospice. Visits may include same day visits for patients newly discharged home, PRN visits in response to patient calls to on-call service, or time specific visits such IV administration, tube feeds, or TPN.

Preferred schedule is 3pm – 10pm Monday through Friday.

Work 35 hours/week get paid for 40 hours. The hourly range is $30 – $34.

Major Responsibilities

1. Ensures patients have timely and accurate orders prior to delivering services. Reviews Physician’s referral (prescription), patient’s condition and medical record.

2. Completes an ongoing comprehensive and interdisciplinary assessment including: OASIS per regulations. Development and ongoing evaluation of individual plan of care based on needs, interests, and abilities in conjunction with physician, interdisciplinary team, patient, caregiver and/or representative.

3. Effectively integrate educational, medical, social and psychological factors when evaluating progress towards attainment of goals and outcomes. Orients, instructs and may direct work activities of licensed practical nurses (LPN), licensed nursing assistants (LNA), and students as appropriate. Evidence of ongoing communication between the nurse, LPN, and LNA regarding patient status and progress must be documented in the patient medical record. RN is responsible for the development of patient LNA plan of care and supervision of LNA services per regulations.

4. Perform services/treatments for patient as indicated in plan of care based on physican orders.

5. Complies with medical, regulatory, educational and records management requirements (e.g. insurance authorization, consents, physician’s referral, documentation) and maintains patient confidentiality in accordance with HIPAA guidelines.

6. Communicates with all physicians and other health care practioners (as appropriate) involved in the patient’s plan of care to exchange, discuss, and evaluate patient information for planning, modifying, and coordinating an appropriate treatment plan and goals.

7. Assess for patient, caregiver, and/or representative and family counseling needs and make appropriate referrals. Provide patient caregiver, and/or representative education including but not limited to: diagnosis, interventions, community resources, medications, safe and therapeutic environment, coping strategies, and discharge, etc.

8. Documents assessment, interventions, medication reconciliation, and discharge planning in appropriate clinical notes and plan of care. Completes documentation same day as visit, per agency standard. Participates in the agency’s quality assessment process improvement (QAPI) program to improve health outcome, patient safety and quality of care.

9. Meets agency obligations for, holiday coverage, weekend coverage, and on call shifts. Complies with Agency standards for productivity, patient and staff schedules, and attendance at meetings.

10. Enhances professional growth and development through participation in educational programs, current literature, community based and home health agency in services, meetings, workshops, and provides training to staff, agency, and community groups. Complies with professional licensure standards and code of ethics.

Position Description:

The Mental Health Registered Nurse (RN) provides direct service to selected patients and families where there are complex emotional and/or psychiatric problems. Following assessment and establishment of a plan, these patients may be carried partially or totally by the Mental Health Nurse. The Mental Health Nurse will assist the health team with concepts in mental health care, will serve on the behavioral health team and serve as a resource for all staff.

The Mental Health Nurse will also carry a regular home care caseload as a Case Manager. The Nurse Case Manager is responsible for independent management of a diverse patient population requiring the use of advanced assessment, teaching and decision-making skills. The nurse is responsible for ensuring that appropriate referrals to other services are made, interdisciplinary conferencing takes place regularly, and appropriate documentation is completed. The Nurse Case Manager cares for a caseload of home health patients by evaluating the patient for appropriateness of home health and developing a patient centered plan of care in conjunction with the physician and interdisciplinary team. The RN provides follow up by evaluating the effectiveness of the home care plan and monitoring patient/family’s response to the plan. The Licensed Practical Nurse (LPN) is a nurse that works under the direct supervision of an RN and performs all assigned tasks that are delegated from the RN. The Nurse educates patients, families, and caregivers to safely perform care in the home. The Nurse has knowledge of home care regulations and third-party reimbursement as it impacts the delivery of services. The Nurse must adhere to their scope of practice and follow all agency administrative, clinical and personnel policies and procedures. This position will have weekend and on-call responsibilities and is under the general supervision of a Team Manager.
