Serves as a liaison for hospitals and other referral sources (physician practices, skilled nursing facilities, and rehabilitation facilities) to grow referrals for Cornerstone VNA (CVNA) services, ensuring high quality care and patient satisfaction within assigned service area. Develops/Builds a strong network of strategic relationships with prospective and under-utilized referral sources, business partners, and members of the community to raise awareness of programs and services at CVNA. Maintains regular contact with referral partners and discharge planners at hospitals, physician practices and other community health organizations to provide program and service education and answer questions regarding patient care. Serves as a patient advocate through visits with current and prospective patients as needed in the hospital, rehabilitation, or other health care facility to monitor patient’s progress and to assist with a safe discharge plan. Collaborates with case management and patient/family members to assure patients understand their home or hospice care benefits and the scope of programs and services available at CVNA. Collaborates with the Program Support Director, Intake and clinical team regarding referrals and inquiries to ensure safe and appropriate care for all patients. Offers educational presentations for prospective new referral partners who have patients/clients in need of VNA services and/or to highlight new programs and services for existing referral partners as needed.
Qualifications for a Community Liaison:
Ability to represent Cornerstone VNA in a trusted and professional manner
1-2 years case management/community liaison experience preferred
3-5 years in health care business development/marketing preferred
Strong communication skills
Exceptional assessment skills
Valid driver’s license in good standing and vehicle insurance required
