Job Description:

Serves as a liaison for hospitals and other referral sources (physician practices, skilled nursing facilities, and rehabilitation facilities) to grow referrals for Cornerstone VNA (CVNA) services, ensuring high quality care and patient satisfaction within assigned service area. Develops/Builds a strong network of strategic relationships with prospective and under-utilized referral sources, business partners, and members of the community to raise awareness of programs and services at CVNA. Maintains regular contact with referral partners and discharge planners at hospitals, physician practices and other community health organizations to provide program and service education and answer questions regarding patient care. Serves as a patient advocate through visits with current and prospective patients as needed in the hospital, rehabilitation, or other health care facility to monitor patient’s progress and to assist with a safe discharge plan. Collaborates with case management and patient/family members to assure patients understand their home or hospice care benefits and the scope of programs and services available at CVNA. Collaborates with the Program Support Director, Intake and clinical team regarding referrals and inquiries to ensure safe and appropriate care for all patients. Offers educational presentations for prospective new referral partners who have patients/clients in need of VNA services and/or to highlight new programs and services for existing referral partners as needed.

Qualifications for a Community Liaison:

Ability to represent Cornerstone VNA in a trusted and professional manner
1-2 years case management/community liaison experience preferred
3-5 years in health care business development/marketing preferred
Strong communication skills
Exceptional assessment skills
Valid driver’s license in good standing and vehicle insurance required
Empathy and compassion

Job Description:

Checks admissions for accuracy of payer information. Obtains initial and follow-up authorizations for commercial insurances and contacts EDS/Medicare/Commercial insurances for eligibility purposes. Familiarity with FISS systems, processes account in suspense, 488 requests and manual adjustments for Medicare billings. Processes and submits Medicare and Medicaid claims either electronically or manually. Reviews and submits commercial insurance claims and private duty claims either electronically or manually. Participates in month-end billing verification. Monitors and follows-up on collections for delinquent accounts. Processes and follows up on miscellaneous billing such as grants, flu billing and Telehealth rental. Provides status reports of accounts receivable to the Billing Manager/Executive Director/Chief Financial Officer as requested. Must work well independently and be meticulous with regard to accuracy, organization and detail. Must demonstrate skill at meeting deadlines and being flexible. Fulfills additional performance responsibilities as assigned and as necessary; accurately and in a timely fashion, as assigned by the Billing Manager. Participates in personal growth and development. Completes work and documentation with accuracy and within agency timeframes. Requires minimal supervision and is self-directed. Protects and honors customer and coworker confidentiality. Meets attendance and punctuality expectations. Demonstrates cooperation with scheduling requests to meet agency needs. Consistently adheres to agency dress code. Demonstrates knowledge of policies and procedures applicable to own job description. Adheres to agency policy and procedures. Maintains agency property, supplies and equipment in a manner that demonstrates accountability. Works towards resolution of interpersonal conflicts as they arise. Skills/Qualifications:

Associates degree in finance/accounting or equivalent experience
Experience in billing, credit, collections, authorizations in a healthcare environment preferred
Experience with MS Office

Must be skilled in use of MS Office and have good written and verbal communication skills

Job Description:

The Homemaker recognizes the importance of their job and makes every effort to be dependable. The Homemaker is responsible for following work schedule and keeping demographics in mind when planning visits, reporting to work on time, no excessive absences, giving appropriate notice for absences. The Homemaker, guided by the individual plan of care, intervenes and observes the client’s environment to promote comfort and cleanliness as well as improve/maintain safety. The Homemaker will use basic math skills to count out money and return correct change to patient when grocery shopping, picking up prescriptions or doing errands. The Homemaker is responsible for attending required meetings, following the individual plan of care, performing job duties well, using appropriate and safe techniques in homemaking to benefit the client as well as self, maintaining compliance with CVNA code of ethics and respects the client’s bill of rights, completing mandatory OSHA in-services and reporting pertinent changes and concerns to supervisor. The Homemaker will adhere to agency policy and procedure. As an integral member of the health team, the Homemaker will seek suggestions to improve one’s own practice as well as to accept responsibility and accountability for their own actions. The Homemaker will be supervised every 60 days by the Life Care Director or designated person, in the home or via phone call to client or guardian. The Homemaker must be trustworthy, have great communication, a positive attitude, treat everyone with respect and be able to identify their own learning needs and seek ways to meet those needs.

A little about us/Benefits:

We service over 90 Veterans in a Non-Medical Home Health aspect. We are looking for someone that is available to jump in when Staff is sick, needs emergency coverage, or is on vacation.
