Homemaker

Perform delegated homemaking duties for patient/clients in their home or other designated facility. Under the general direction of the Clinical & Staff Development Manager and the direct supervision of the Registered Nurse and within established policies and procedures.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES

Perform homemaking duties for patient/clients in their home or other designated facility including light housekeeping, light food shopping, errands, pick up medications from pharmacy and meal preparation.
Report and document patient/client’s needs and responses to the appropriate Case Manager and/or Manager. Report patient/client’s change in condition or situations immediately.
Adheres to patient/client Home maker care plan as set up by the primary nurse/therapist while maintaining a healthy, safe environment.
Provide non-personal care while promoting client/patient dignity and provide a calm, supportive and therapeutic environment for client/patients and their families.
Provide excellent customer service to patients/clients.
Utilize supplies and equipment in a cost-effective manner.
Document adequately to ensure prevention of write-offs.
Refer questions regarding costs of care or insurance coverage to appropriate Case Manager or Clinical Manager.
Perform other duties as assigned.
Adhere to guidelines that prohibit the following tasks from being performed in this role including:
Heavy seasonal house cleaning such as washing floors, curtains, woodwork;
The family wash or ironing;
Dressmaking or tailoring;
Lending or borrowing money or handling of any family business affairs;
Administration of medications;
Give personal address or telephone number
Cannot transport any clients in their car.

A little about us/Benefits:

We service over 90 Veterans in a Non-Medical Home Health aspect. We are looking for someone that is available to jump in when Staff is sick, needs emergency coverage, or is on vacation. This could be last minute (sick/emergency) or planned in advance (vacation coverage). We service Veterans all over the State, so travel would be a must. We offer a gas card to help alleviate those travel costs.

If you are looking for some part time hours to supplement the Per Diem hours, we can often place you with a Veteran close to your home.

The Home Care Quality Specialist is responsible for the successful review of clinical documentation including OASIS, 485s, Interim Orders on a daily basis. Provides subject matter expertise and key Quality resources for Home Care Leadership, Clinicians and Office Personnel. Provides support and guidance regarding regulatory, compliance issues and best practices for Home Care services. Mentors and supports Clinicians by leading, engaging and motivating them to provide exceptional care while constantly improving processes, outcomes and patient satisfaction.

 

This Specialist will help to establish and maintain Home Care policies, procedures and activities to meet the standards established by CMS, JC and other regulatory organizations.

 

The Specialist will work with the Quality Improvement Manager to review, prepare and submit all requests by the fiscal intermediary for Additional Documentation Requests (ADRs) as well as other entities such as MAC, RACs, ZIPs, CERTs and Surveying entities. In collaboration with the Quality Manager successfully implements Medicare CoPs, continually improves 5 Star Ratings, develops/implements/monitors plans for compliance with Joint Commission and other regulatory standards.
