Hospice House LNA – Full Time

The Hospice House Aide helps achieve our mission by delivering age appropriate quality of life care to residents in facility settings. The Hospice House Aide will effectively interact with residents, family members, facility staff and other healthcare team members. Some essential functions include:

Follows plan of care in providing basic care and services to the resident including assistance with Activities of Daily Living (ADL): walking, bathing, shaving, mouth care, grooming, eating transfers, and management of incontinence.
Communicates with nurses involved in the resident’s care plan; observes, records and reports changes in resident’s condition, status, or other pertinent information to the nursing staff
Laundry, housekeeping and maintenance to assure a safe, orderly, clean and comfortable environment.
Provides supervision to residents with cognitive deficit.
Ensures residents have the opportunity to engage in cultural, spiritual, physical, political, social and intelligent activities as the residents’ desire.

Home Care – LNA

Granite VNA is hiring Home Health Aides! This is a per diem job opportunity. Hours are typically from 9 a.m. – 1 p.m. Monday – Friday. Hours and days per week are flexible.

Follows care plan in providing basic care and services to the client including assistance with Activities of Daily Living (ADL): walking, bathing, shaving, mouth care, grooming, eating transfers, and management of incontinence.
Communicates with nurses involved in the resident’s care plan; observes, records and reports changes in resident’s condition, status, or other pertinent information to the nursing staff clearly, concisely and in a timely manner.
Documents care provided to the resident accurately in accordance to the agency’s guidelines.
Complies with the Agency visit documentation, time entry and electronic scheduling requirements.
Assists client with homemaking tasks, medication assistance, and wound care according to the patient’s care plan.

Physical Demands
This position requires the ability to frequently enter and exit and/or sit in an automobile for prolonged periods while commuting to work assignments.
Ability to talk and hear; read, write and speak in English, normal or correctable vision, and hearing.
Intermittent physical activity, including standing, walking, bending, kneeling, stooping and crouching as well as lifting and supporting patients.
Ability to minimally lift up to 50 lbs. and the ability to provide up to 75% assistance for patient transfers.
Ability to frequently enter and exit and/or sit in an automobile for prolonged periods while commuting to work assignments

Personal Care Service Professional

We are seeking a personal care service professionals for our non-medical home care business. We offer caring home assistance services for seniors and the disabled need to maintain an independent life style with the comfort that staying in their own home provides.

From helping with meals, assisting with personal care, to providing companionship for the elderly, disabled, and recently discharge patients.

Responsibilities may include the following:
– Have a caring heart and the desire to help others
– Have a valid Driver’s License, dependable vehicle and auto insurance

– Ability to travel to client’s houses in and around Merrimack county and within 45 miles of Concord

Partial/Full Live-In Home Care Provider

We are a non-medical personal home care company that has an interest from one of our clients for a full-time live-in care giver.

We will not be able to take them on until we have filled all the position(s). We are open to scheduling, but the stated rate in this post would be for a 72-hour shift, or $150 per day. We initially considering three hires for the three shifts (two 72-hour shifts and one 24-hour shift). However, if we find the right candidates and they have a desire to work a 96 hour shift or an entire week, that can be arranged as well.

Client needs hands-on care for about 4-5 hours per day, the rest would be companion services and home making. You also would have a private bedroom, where you can sleep nightly, but a shared bathroom.

At the end of each shift, you would need to prepare the room for the next caregiver.

Candidate needs to be responsible and reliable. We know everyone says that, but because of the length of the shifts, and the type of care, we cannot afford to have someone calling out last minute. Any changes in scheduling will be difficult to fill and we understand it takes a certain personality to fill the offered position.
