Medical Social Worker – Home Care

Current Opening: Monday – Friday, FT 37.50 – 40 hours or PT hours available. Opening available in the Laconia area.

Reporting to the Home Care Director, the Granite VNA Medical Social Worker (MSW) provides age appropriate support, education and counseling to the patient and family members in the home setting utilizing standard social work practice. MSWs will effectively interact with patients, care givers and other healthcare team members while maintain all standards of professional practice.

Essential Functions

Develops and implements appropriate plan of care (POC) that complies with referral source orders, specific treatment measures, frequency/expected duration and discharge goals in partnership with the patient, healthcare provider, patient representative (if any) and/or caregiver(s) based on thorough assessment incorporating all aspects of patient limitations and goals, and potential discharge environment.
Documents accurate and ongoing assessment, treatment and discharge of the patient: interventions, patient response to care provided, patient needs, problems, capabilities, limitations and progress toward goals. Documentation includes evidence of appropriate patient/caregiver teaching, and the understanding of these instructions in accordance with agency timeframes.
Engages the patient /caregiver in meaningful education as indicated per care plan.
Communicates with all physicians involved in the POC and other health practitioners related to the current POC.
Demonstrates responsibility to effectively coordinate the patient’s POC with members of the interdisciplinary team and other health professionals through team/patient care conference. Updates patient’s short-term/long-term goals as appropriate based on interdisciplinary team assessments and re-assessments.
Participates in interdisciplinary team, agency projects, committees, QAPI program and/or other activities upon request and approval from direct supervisor.
Attends Agency In-service programs.
Other Job Functions

Coach, mentor and precept new medical social work employees and students.

Job Description:

The social worker will complete a comprehensive and culturally competent assessment of the client’s psychosocial, environmental and financial needs. Which includes appropriately adding SW pathway and involving client, family and other health care practitioners in assessment. Develops goal plans that incorporate assessment data, identify expected outcomes and enhance the clients’ ability to manage their disease process or disability. Develop individualized pathways and revise based on pt/cg needs, resources, finances and psychosocial deficits. Ensures patient is scheduled out correctly using correct codes. Communicates appropriate data to other persons involved in the individual’s or family’s care such as other team members and/or other health providers. Social worker will develop goal plans that incorporate assessment data, identify expected outcomes and enhance the clients’ ability to manage their disease process or disability. Social worker utilizes appropriate intervention strategies to assist the home health client to meet their goals. Adapt intervention strategies to work effectively with clients from different age groups, cultures, socioeconomic backgrounds and differing states of mental health and disability. Documentation will be done in the home or prior to the next patient. Correct template will be used to document coordination notes. Orders are done correctly including spelling, description, when to send or not to send, correct scheduling codes, etc.). Patients are prepared for discharge in accordance with the NASW Code of Ethics and Cornerstone VNA discharge process. The social worker will demonstrate knowledge of the available community resources which may be engaged on behalf of any client of the home health agency. Establish and maintain professional relationships within the community. Advocate and educate clients and families, as appropriate, to access resources and implement services. System of providers is accessed through network and data base resources which is maintained and updated by the social work department. The social worker evaluates his or her own social work practice in relation to professional practice standards. The social worker engages in performance self-appraisal on a regular basis, seeks constructive feedback from peers and management, identifying areas of strength as well as areas for professional development. Effective preceptor to new employees. Participates in educational activities in-house and office to maintain NASW licensing. Actively participates in the agencies Quality Improvement Performance Improvement (QAPI) plan and projects. Seeks opportunities in the agency and/or community to provide education or improvement in care initiatives. The social worker is responsible for adhering to agency policy and guidelines concerning completion of time logs accurately, tact, attitude, punctuality, attendance, discretion, dependability, and productivity.

Licensed Nursing Assistant – Hospice – Full time

Position Description:
We are seeking to hire a Licensed Nursing Assistant (LNA) to provide trusted, compassionate and expert home care to our patients in our Hospice Program.


Licensed Nursing Assistants (LNAs) will work under the supervision of a licensed nurse and be responsible for assisting with direct client care. He or she will be responsible for performing the delivery of client/patient care, including meal preparation, feeding, toileting, grooming, respite, and light housekeeping. He or she will adhere to established policies and procedures and maintain a clean and safe environment for client/patients. The LNA must be listed as current on the NH Board of Nursing website.

Educational Requirements:

Licensure/Certification in New Hampshire. Hospice experience preferred.

Prefer candidate to be eligible for ME CNA registry.

Perform general social work services that endeavor to maintain, strengthen and safeguard home and family life where there are social and/or health needs that interfere with independent functioning of individuals or families. Provide services to people regardless of their ability to pay, within the resources available.

Provide rehabilitative and supportive casework geared toward restoring patients to their optimum level of social and health adjustment including assisting patients and their families in understanding, accepting, and following medical recommendations.
Help patients utilize the resources of their families and the community including referring patients to resources or acting as an advocate on behalf of the patients in their dealings with other health and welfare agencies.
Assist patients and their families with personal and environmental difficulties, which predispose them toward illness or interfere with obtaining maximum benefits from medical care to include counseling members of the patient’s family to assist patients with admission to a nursing home.
Consult with the physician and other members of the healthcare team for the purpose of assisting them to understand significant social, emotional, and environmental factors related to the patient’s health problems.
Prepare clinical/progress notes on the day of the visit and incorporate same in the clinical record and provide summaries and re-evaluations as applicable.
Attend case conferences and IDT.
Participate in staff development and revision of the physician’s plan of treatment.
Participate in staff development activities and in-service education.
Participate in discharge planning and in-service programs.
Act as a consultant to other agency personnel.
Communicate effectively with all healthcare providers.
Confirm, on a weekly basis, the scheduling of visits with the Case Manager to coordinate necessary visits with other personnel.
Provide notifications that affect agreed services with the company.
Provide services on a short-term basis (two to three visits) to a beneficiary’s family member or caregiver when it is shown that a brief intervention is necessary to remove a clear and direct impediment to the effective treatment of the beneficiary’s medical condition or to the rate of recovery.
Take appropriate action to obtain available community resources to assist in resolving the beneficiary’s problem.
Perform other duties assigned.



Master’s degree from an accredited school of social work approved by the Council of Social Work Education.
Current MSW license to practice Social Work in the State of New Hampshire is required.
Minimum of 1-year experience as a Social Worker, preferably in a home healthcare setting.


Ability to assess social and emotional factors related to the beneficiary’s illness, the need for care, their response to treatment, and adjustment to care.
Ability to assess the relationship of the medical and nursing requirements to the home situation, financial resources, and the community resources available.
Ability to work independently and take initiative.
Good judgment and problem-solving skills.
Ability to demonstrate courtesy, compassion and respect in relating to patients, families, visitors, physicians and co-workers.
Ability to work as a team and maintain positive working relationships with employees and healthcare professionals.
Ability to document patient care accurately according to healthcare standards and plan of care.
Working knowledge of Microsoft Office software and other industry specific programs.
Must demonstrate ethical conduct and build trust with patients and family members.
Ability to follow safety guidelines as instructed by the agency.
Ability to wear protective clothing such as mask, glove, head cover, safety glasses, etc.
Ability to travel required. Travel throughout central New Hampshire is required.
Current drivers’ license is required.
Proof of automobile insurance is required.
Ability to pass a motor vehicle background screening is required.


Job requires physical activities including but not limited to; standing for extended periods of time, bending, lifting, reaching, kneeling, pushing, pulling, crouching, fingering, grasping, feeling, talking and hearing.
Job requires light physical work including the ability to exert up to 20 lbs. of force occasionally, 10 lbs. regularly.

Hospice & Palliative Care Social Worker
