Speech Therapist

Pemi-Baker Community Health is seeking an experienced, self-motivated, and passionate speech pathologist/therapist to join our existing physical and occupational therapy home health program. Position would be Per Diem, as needed.

Essential Primary Duties:

*Evaluate and diagnose speech, language, communication, or swallowing difficulties.
*Develop and implement effective treatment programs.
*Provide counseling to patients and patients’ families about results of testing and recommended treatment options.
*Establish patient understanding, expectations, and goals for treatment.
*Create accurate documentation and reporting for all patient visits.
*Maintain communication with staff and outside agencies per state guidelines.
*Maintain patient confidentiality at all times, conforming to HIPPA privacy regulations.

Prerequisite Skills:
*Knowledge of Medicaid, Medicare, and private insurance plans.
*Familiarity with a variety of speech and cognitive disorders.
*Strong interpersonal and communication skills.
*Ability to work collaboratively with a team (PT & OT).
*Basic computer literacy.
*Ability to comply with all regulatory standards.

Physical Therapist

Responsibilities
• Assess each patient and develop an appropriate treatment plan with measurable goals and time frames

• Ensure that other treating medical professionals have the necessary information to appropriately evaluate the patient

Occupational Therapist

We are looking for a committed and patient-focused Occupational Therapist to develop, recover or maintain the daily living and work skills of people with physical, mental or developmental conditions. You will offer effective and client-centered services that will enable people to confidently and independently carry out everyday tasks or activities. The successful candidate will have an holistic perspective and will be able to adapt the environment to fit the person.

Responsibilities

Address client’s aspects of performance to support engagement in occupations that affect health, well-being and life quality
Select and adapt the appropriate per case occupational therapy treatment model, method and approach to direct the process of interventions
Apply interventions to maximize safety and performance in activities of daily living (ADL) and instrumental activities of daily living (IADL)
Teach clients new ways of approaching tasks and educate them on how to break down activities into achievable components
Assess client’s home and other environments and advice on alterations consistent with their needs and capabilities
Recommend adaptive equipment and train in its use
Guide and educate family members and caregivers
Observe, note, and report on progress of long and short term treatment goals
Collect data and document processes followed
Regularly follow-up client

Speech Language Pathologist $1500 sign on bonus

As a SLP have you ever considered providing patient care in their home which offers a unique perspective to their overall health? We have a great opportunity to provide Speech Language therapy to patients in their homes on a part time basis 20 hrs. a week with $1500 sign on bonus!

The Speech Therapist will determine speech language pathology treatment plans in consultation with physicians and homecare team. The therapist will also develop interventions to support goals of therapy and restore maximum function and strength promoting optimal health. A minimum of two year’s experience evaluating and treating children and adults with speech, language, nuerological and/or swallowing disorders is required.
Therapist must have a Master’s degree in Speech Language Pathology or Communications Disorders, have a certificate of Clinical Competence in speech-language pathology and be registered in NH as SLP. Position offers flexible schedule and hours primarily on the weekdays. We offer competitive pay and working 20 hrs. a week is eligible for paid time off!
