Ensures delivery of quality client/patient care by creating a work atmosphere conducive to professional growth, accountability and autonomy. Ensures staff meets goals in areas of quality service, and cost as defined by the organization’s mission, vision and values and strategic initiatives. Acts as a resource to staff. Ensures that the staff are in compliance with regulatory and organizational standards. Is responsible for the clinical coordination, case management, and care planning oversight that will achieve goal-driven, team- based care. KEY RESPONSIBILITIES Provides direct supervision and oversight for interdisciplinary home care clinicians. Direct supervision includes case conferencing weekly Ensures interdisciplinary staff members are clinically competent and technically proficient with use of the electronic medical record. Based on patient acuity/complexity needs, develops and implements staff education, policies and procedures Thorough understanding of all applicable federal and state regulatory and compliance standards, Elliot system policies, procedures and strategies. Provides input into VNA strategic planning, quality improvement, policy development and system design processes. Performs similar or related duties as assigned or directed. Responsible for assuring the development, implementation, and updates of the individualized plan of care to include the following: • Making patient and personnel assignments in collaboration with team assigned Scheduling Coordinator • Coordinating patient care working in collaboration with Scheduling Coordinator • Coordinating referrals to assign for admission to ensure timely initiation of care • Assuring that patient needs are continually assessed • Assuring the development, implementation, and updates of the individualized plan of care • Review and oversight of LUPA and utilization management • Promotes communication among the team to include daily huddle to review the day’s assignment • Ensuring productivity standards are met per discipline • Reviewing clinical scorecard/outcomes on a monthly basis with each clinician REQUIRED KNOWLEDGE & SKILLS Ability to work independently and take initiative; Good judgment, critical thinking, coaching and problem-solving skills Ability to assess, problem-solve and make appropriate decisions in a fast-paced environment. Ability to communicate effectively and facilitate communication between and among others. Ability to demonstrate courtesy, compassion and respect in relating to client/patients, families, visitors, physicians and co-workers Ability to demonstrate flexibility and the ability to adapt to change Ability to establish a cooperative, collaborative working environment and maintain positive working relationships with employees, physicians, referral sources, clients/patients, families and the community
Qualifications EDUCATION/EXPERIENCE/LICENSURE Education: Graduate of an accredited school of nursing and/or accredited school for physical or occupational therapist required. Experience: Five years of progressive relevant professional experience with two years in management/supervisory position preferred. Certification/Licensure: Current licensure as a Registered Nurse, in the state of New Hampshire required or current licensure for Physical/Occupational Therapist.
