Overview Clinical Triage Nurse - Full time Reporting to the Director of Operations, the Clinical Triage Nurse provides telephone triage for calls from home care and hospice community patients and families, skilled and assisted living nursing facility staff, John H Whitaker Place, physicians and their office staff, pharmacy, durable medical equipment providers and other type calls related to active patients. Duties will include assisting both the home care and hospice programs with clinical triage for home health calls and visits. Essential Functions Clinically triages phone calls from patients/families/caregiver by providing skilled and palliative care assessment, problem solving and instruction along with assisting to coordinate home visits as indicated for patients who call the agency seeking guidance. Clinically coordinates the scheduling of complex IV, Medication Reconciliation and Wounds Care referrals. Provides guidance including pain management and symptom control; conducts assessments and evaluations and provides education and support care to patients and caregivers in collaboration with the nurse case manager. Coordination of patient care with the physician office and enters orders as indicated. Communicates outcome with clinical team. Receives and contacts by phone/fax/email any patient physician (designee) as indicated by the change in the patient’s condition, treatment and/or status. Accepts and completes all necessary documentation related to accepting medications and/or Durable Medical Equipment for Hospice patients. Completes comprehensive documentation about the calls received/initiated in the patient’s medical record using the SBAR guide. Assists daily with clinical triage of community and facility nursing and therapy visits and admissions for both home care and hospice programs. Supports scheduling with determining priority of assigning open visits. Assists with chart review and monitoring of Quality Improvement and Patient Safety activities as requested. Reviews orders received by intake that impact the patient’s plan of care and makes appropriate changes. Acts as a resource to the Home Health and Hospice clinical team. Demonstrates flexibility in the role by accepting other duties as assigned for which qualified. While performing the duties of this job, the employee is regularly required to talk and hear. Specific abilities include reading, writing and speaking in English, normal or correctable vision with the ability to accurately discern colors, and hearing. This position requires the ability to use a computer to document patient care. Specific abilities include use of fine motor skills to type, write and perform professional skills. This position requires intermittent physical activity, including standing, walking, bending, kneeling, stooping and crouching. This position requires the ability to minimally lift to 25 lbs. This position requires the ability to sit for prolonged periods.
Qualifications Graduate of an accredited school of nursing, accompanied by current New Hampshire nursing license, or compact state license through the Nurse Licensure Compact (NLC). One-two years of home health or hospice experience. One-two years of telephone triage experience. Work Environment The employee will be exposed to a controlled work environment. The employee may be required to work extended periods of time using a desktop, laptop and/or other mobile device.
