Overview Community Benefit RN Are you a Registered Nurse looking for a flexible schedule or looking to reduce the amount of hours you work per week? This position may be the perfect fit for you! This position is primarily located in the Lakes Region. Approximately 10-12 hours per week. Opportunity for more hours! The Community Benefit RN establishes, monitors and delivers foot care to clients in a clinic setting as per the nursing care plan and in accordance with all required internal and external professional practice standards. RNs will effectively interact with patients, care givers and other healthcare team members while maintaining all standards of professional nursing care. Essential Functions Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. Completes assessment and reassessment of clinic patients at each encounter to identify problems and/or progress toward desired goals. Reports changes in the patient's condition, as necessary, to the attending physician or Granite VNA Intake Office. Provides nursing care for patients utilizing acceptable standards of care and within scope of practice. Records statistics on clients seen and submits to the Community Health Program Assistant monthly for the purpose of Community Benefit Reporting. Demonstrates responsibility to effectively communicate with and refer to members of the Granite VNA Home Care and Hospice Team, and other health professionals as needed. Attends Agency In-service programs. Other Job Functions Actively participate in team/staff meetings or review agenda and minutes and provide constructive input to the meeting leader. Identify problems and offer constructive suggestions/solutions. Identifies and communicates concerns regarding patient safety issues to Community Benefit Manager. Demonstrates flexibility in role by accepting other duties as assigned for which qualified.
Qualifications Graduate of an accredited school of nursing, accompanied by current New Hampshire nursing license, or compact state license through the Nurse Licensure Compact (NLC). One year of recent general medical/surgical or skilled/acute care nursing experience. Proof of completed COVID-19 Vaccine.
