| Overview | Position Description The primary role of this position is to facilitate excellent communication between staff and our multiple referral sources, providing enhanced discharge planning and provide a smooth transition to home for potential/active clients. She/he will maintain and build positive relationships with hospitals, physician offices, skilled nursing facilities, rehabs and other types of alternative living sites. She/he will offer education to the community regarding the benefits of hospice services. Educational Requirements RN Degree/NH/ME Other Requirements Three years of hospice and/or homecare experience preferred Skilled Nursing facility and/or Assisted Living facility experience preferred Strong communication skills Exceptional assessment skills Proficiency with medical EMR systems Valid driver's license in good standing and vehicle insurance required Ability to provide trusted, compassionate and expert care Benefits Medical/Dental/Life Insurance On-going education/training Generous earned time benefits Tuition reimbursement AAA Coverage Mileage Reimbursement Exceptional and supportive work environment Flexible scheduling For more information, or to apply now, you must go to the website below.
