Overview HCS is seeking a Hospice Program Care Manager. This compassionate professional will support the patients and staff of the Hospice at HCS program. This is a full-time, salaried position located at HCS' Keene, NH office. The Hospice Patient Care Manager (under the direction of the Hospice Program Director and as a member of an interdisciplinary care team) plans, provides, directs and evaluates nursing care in any or all of the agency’s programs, as well as offers instructions and guidance in health practice for individuals and families. Home Healthcare, Hospice and Community Services (HCS) is a non-profit organization providing comprehensive services in southwestern New Hampshire. Qualifications Qualifications needed: Graduate of a National League for Nursing approved program; Preparation in community staff nursing through a National League for Nursing approved program; Bachelor of Science in Nursing degree; and Three (3) years of professional nursing experience with some supervisory level of experience (or a combination of education and experience that demonstrates the necessary competencies for the position.) Other requirements of this position include: Possession of a current State of New Hampshire license to practice as a registered professional nurse; Maintain current Cardiopulmonary Resuscitation (CPR) certification; Have a valid New Hampshire driver’s license with unlimited access to reliable transportation; Have adequate automobile insurance; Demonstrate sensitivity in human relationships (coping with family emotional stress, tolerant of individual lifestyles, skill in using psychological principles in both one on one and group situations, knowledge of measures to comfort the dying, and the emotional stability, flexibility and the ability to accept various religions and spiritual beliefs); Must be willing to complete an approved Hospice Volunteer Training program; Hospice/Palliative Care and/or Oncology Certified, or demonstrate willingness to obtain certification; IV certification and current IV Therapy experience, or ability to become IV certified; Able and willing to learn to utilize a portable electronic field device; and Able to manage electronic patient health information (EPHI) remote access in accordance with HCS' policies and procedures.
