Overview HCS is seeking a Hospice Program Director. This compassionate professional will foster and strengthen HCS' commitment to provide holistic, patient-centered, end-of-life care to patients in their homes throughout southwestern New Hampshire. This is a full-time, salaried position located at HCS' Keene, NH office. Under the direction of the President/CEO, the Hospice Program Director leads the critical operations of HCS' Medicare Certified Hospice Program. Responsibilities for this role include: recruiting and assisting with the training of program staff; evaluating the program and staff performance; engaging referral sources and the general public about the services of Hospice at HCS; preparing and managing the program budget in collaboration with the Fiscal department; and ensuring compliance with legal, regulatory and quality assurance requirements. As a member of the Senior Management Team at HCS, the Hospice Program Director actively participates in agency leadership, management, planning and evaluation. Additionally, the Hospice at HCS Program Director takes part in agency-wide functions as appropriate, attends Board meetings, rotates in the Director-on-call schedule, and is present at all meetings and committees where Senior Management representation is required. Qualifications The ideal candidate will possess the following credentials and characteristics: graduation from an accredited school of nursing or social work; a master’s degree in Nursing Oncology, Administration, or Social Work; Hospice/Palliative Care certification; five (5) years' experience in Hospice with two years in hospice management (or a combination of education and experience that demonstrates the necessary competencies for the position); the ability and desire to work as part of an interdisciplinary team; organizational skills relevant to linking the hospice program to the community; excellent verbal and written communication skills; A valid New Hampshire driver’s license, adequate auto insurance, and unlimited access to reliable transportation; able and willing to learn to utilize a portable electronic field device; and able to manage electronic patient health information (EPHI) remote access in accordance with HCS policies and procedures.
