Overview JOB SUMMARY Delivers care to patients and families with needs ranging from basic to complex utilizing the nursing process. Creates a therapeutic and caring environment for patient and families. Practices in a manner consistent with the Franklin VNA & Hospice Practice model; the ANA’s Social Policy Statement, Scope and Standards of Practice, and the Nursing Code of Ethics; and relevant specialty standards of practice. Assists the Director & CEO with Hospice supervisor tasks and runs IDT meetings. Requirements ESSENTIAL FUNCTIONS AND RESPONSIBILITIES Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. Assists the Director & CEO with Hospice supervisor tasks. In charge of IDT meetings. Provide Hospice education to staff. Explain hospice services and benefits to referral sources, patients, family members and significant others. Ensure there are physician orders prior to delivering services. Complete a comprehensive assessment including physical, psycho/social, and spiritual needs related to the terminal illness that must be addressed in order to promote the patient’s well-being, comfort and dignity. Review available clinical data from referral and lab results, etc. as part of database. Document data and assessment on appropriate form according to guidelines. Reassess client status at established time frames and as indicated by the client status. Reports status and changes in status as appropriate. Assess potential for development of crisis situations based on combination of subjective and objective cues. Develop an individualized approach to deliver nursing care to the individual client and family. Collaborate with interdisciplinary team to develop an integrated plan of care. Orders patient supplies in appropriate quantities and in a timely manner. Assess need for referrals to other disciplines and outside resources. Acknowledge, transcribe, coordinate and implement the medical diagnostic and therapeutic orders. Promote client dignity during provision of care. Provide calm, supportive and therapeutic environment for clients and their families. Assess the client response to and understanding of care provided. Case manage clients in collaboration with other disciplines. Act as a resource to nursing students in provision of client care. Review and update the plan of care in collaboration with the client/patient/family and other Interdisciplinary team members. Provide education to staff and facilities regarding client care. Act as a primary case manager when requested. Participates in interdisciplinary team, agency projects, committees, QAPI program and/or other activities upon request and approval from direct supervisor. Perform other duties assigned.
