Overview Provide basic patient care under direction of the RN and Clinical & Staff Development Manager. Perform duties such as taking and recording vital statistics, feed, bathe, dress, groom, move patients and change linens. May also perform light Homemaker duties. ESSENTIAL FUNCTIONS AND RESPONSIBILITIES Adhere to patient/client LNA care plan as set up the while maintaining a healthy and safe environment. Reports and documents patient/client’s needs and responses to the Nursing supervisor. Reports patient/client’s change in condition or situations immediately. Provide physical support to assist patients to perform daily living activities, such as getting out of bed, bathing, dressing, using the toilet, standing, walking, or exercising. Turn or reposition bedridden patients. Record vital statistics or other health information. Review patients' dietary restrictions, food allergies, and preferences to ensure patient receives appropriate diet. Measure and record food and liquid intake or urinary and fecal output, reporting changes to medical or nursing staff. Monitor patients to detect health problems and advise Nursing supervisor. Perform light housekeeping and Homemaker duties deemed acceptable by the agency in conjunction with or in the absence of the agency Homemaker. Perform other duties assigned.
Qualifications EDUCATION AND EXPERIENCE High School diploma or equivalent and graduation from an accredited Licensed Nursing Assist program. Current Certification/Licensure for State of New Hampshire is required. Minimum of 6 months of experience in an LNA or related role required. SKILLS AND ABILITIES Excellent patient care skills General knowledge of nursing and health care terminology. Ability to demonstrate a caring, compassionate and empathic manner with patients and family members. Maintain positive working relationships with employees and healthcare professionals. Strong verbal communication skills. Ability to document patient care according to nursing standards and plan of care. Working knowledge of Microsoft Office software. Must demonstrate ethical conduct and build trust with patients and family members. Ability to follow safety guidelines as instructed by the agency. Ability to wear protective clothing such as mask, glove, head cover, lead apron, safety glasses, safety shoes, etc. Ability to travel required. Travel throughout central New Hampshire is required. Current drivers’ license is required. Proof of automobile insurance is required. Ability to pass a motor vehicle background screening is required.
