Overview | Sign on bonus: $4000 The Palliative Care Registered Nurse (RN) Coordinator is responsible for overseeing the care of patients in our Palliative Care Program. The Palliative Care RN Coordinator will provide palliative care to patients and their families in the home and facility setting, including assisted living, intermediate care and long term care facilities. Strong clinical support and educational resources, including a structured orientation with your own preceptor Work in an environment where agency culture fosters teamwork and excellence No night or weekend scheduling Responsibilities of Palliative Care Registered Nurse (RN) Coordinator: Assist patients and families with navigating through serious illness management and end of life planning through goals of care discussions Assess needs and provide palliative nursing interventions including symptom management recommendations Manage a case load of palliative patients in the community with the assistance of the palliative care team, which includes a physician, medical social work and spiritual care support. Collaborate with referring providers in the community to achieve high quality patient care Provide education and outreach to promote palliative care in the community Lake Sunapee Region VNA is a mid-sized Homecare, Hospice, and Private Duty agency covering 32 towns and 3 counties in the Lake Sunapee and Upper Valley regions. We provide patient centered care in the home setting with the goal to keep patients safely in their home for as long as they desire. Lake Sunapee Region VNA provides hospice services with the goal of providing the patient with the end of life experience they desire to have. Qualifications | Requirements for Palliative Care Registered Nurse (RN) Coordinator: Active NH Registered Nurse (RN) License At least 1 year of Hospice or Palliative Care experience Preferred: Active NH Certified Hospice and Palliative Nurse (CHPN) |
