Overview LRVNA is hiring a full time Physical Therapist The Home Health Physical Therapist is responsible for the evaluation and treatment of patients with orthopedic, neurological, complex-medical and other diagnoses that affect a person’s strength, range of motion, endurance and overall function. The Physical Therapist will utilize therapeutic exercise, modalities, and activities to achieve the highest level of function possible. Desired Skills and Experience REQUIREMENTS: The qualifications for the Physical Therapist are: Must be a graduate of an American Physical Therapy Association accredited program and hold minimally a Bachelor’s degree, preferably a Master’s Degree in Physical Therapy; Must hold a current and valid Physical Therapy practice license granted from the New Hampshire Office of Allied Health Professions. Competent in Computer use. ESSENTIAL RESPONSIBILITIES: - Required skills and competencies. - Responsible for an initial evaluation, including objective goals, of patients referred to physical therapy prior to beginning treatment. Treatment will occur only with a written referral from a health care provider with approved privileges to prescribe physical therapy services as set forth in the State of New Hampshire Physical Therapy Practice Act. - Responsible for developing a written treatment plan of care in consultation with and approval from the referring provider. - Responsible for maintaining current progress notes in the patient’s record. Patient progress in relation to goals to be noted, type of treatment rendered and evidence of periodic reviews of the referring provider also to be noted in the record. - Ensures that all documentation is legible and, in the format, approved by the department: SOAP notes for daily visits and a narrative format for an initial evaluation is preferred. - Ensures that equipment used by the patient is in safe working condition prior to use by the patient and that any unsafe equipment will be reported to the Clinical Nurse Manager and Maintenance. - Maintains patient supplies within Home Health in reasonable amounts for patient provision. - With the Clinical Nurse Manager, develops and updates policies and procedures relative to physical therapy practice. - Attends meetings as designated by the Clinical Director. - Works collaboratively with the Primary Nurse and Rehab Manager. - Demonstrates knowledge and skills necessary to provide appropriate care within the patient’s home. - Demonstrates knowledge of appropriate modification of treatment programs and safety principles for providing care within the home. - Manages time effectively to ensure that deadlines are met and that personal priorities support organizational objectives. - Performs all other duties as assigned.
