Overview Current Opening: Full-time 37.50 hours per week, Monday - Friday; 8 a.m. - 4 p.m. Openings available in Manchester area and areas West of Concord. The Granite VNA Physical Therapist Assistant (PTA) delivers age appropriate care to the patient in the home setting utilizing standard therapy best practice: implementation of plan of care as established by a Physical Therapist, and within scope of practice defined by the NH Physical Therapy Governing Board. PTAs will effectively interact with patients, care givers and other healthcare team members while maintaining all standards of professional therapy. PTAs are supervised by the Physical Therapist assigned to the patient. Essential Functions Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. Follows the appropriate plan of care (POC) developed by the Physical Therapist in partnership with the patient, healthcare provider, patient representative (if any) and/or caregiver(s) incorporating all aspects of patient limitations and goals. Documents accurate and ongoing treatment of the patient: therapy interventions, patient response to care provided, patient/caregiver educational needs, problems, capabilities, limitations and progress toward goals. Documentation includes evidence of appropriate patient/caregiver teaching, and the understanding of these instructions in accordance with agency timeframes. Engages the patient/caregiver in meaningful education as indicated per care plan. Communicates with all physicians involved in the POC and other health practitioners related to the current POC. Demonstrates responsibility to effectively communicate the patient's achievement of goals in the POC with members of the interdisciplinary team and other health professionals through team/patient care conference. Participates in interdisciplinary team, agency projects, committees, QAPI program and/or other activities upon request and approval from direct supervisor. Attends Agency In-Service programs. Qualifications Graduate of an approved physical therapist assistant education program (CAPTE), accompanied by current New Hampshire physical therapy assistant license. One year of physical therapy experience. Proof of completed COVID-19 vaccine
