Overview ob Summary At North Country Home Health and Hospice, the Physical Therapist is an integral part of the patient care team. He/she is responsible for the comprehensive assessment, treatment, and case management of patients with orthopedic, neurological, complex-medical, and other diagnoses that have led to impairments in the patient’s overall functional ability. The PT will utilize a wide range of skill sets, working in the patient’s home environment, to achieve the highest possible outcomes outlined in the patient-centered plan of care. Essential Responsibilities: Performs comprehensive assessments or evaluations leading to the development of a patient centered plan of care in accordance with CMS guidelines of the Conditions of Participation for Home Health Care Maintains accurate and timely documentation in the patient’s electronic record of treatment rendered, progress toward goals, adjustments in treatment as needed and a safe and appropriate discharge plan. Communicates effectively with patients, caregivers, providers, members of the care team and the NCHHHA management team Possesses the ability to work effectively both autonomously and within a team environment Attends and actively participates in designated meetings Demonstrates the knowledge and skills necessary to provide appropriate care within the patient’s home. Demonstrates knowledge of appropriate development and modification of treatment programs including safety principles for providing care within the home. Manages time effectively to ensure that deadlines are met and that personal priorities support organizational objectives.
Qualifications A graduate of an American Physical Therapy Association accredited program Holds, at minimum, a bachelor’s degree in Physical Therapy, Masters or higher preferred Possesses a current and valid New Hampshire Physical Therapy license Proficient in computer use and electronic medical records Competent in the use of Microsoft office products 1 year working as a physical therapist required, 3+ years preferred Home Health experience preferred
