We are currently recruiting for an RN Intake Coordinator to join our team. The intake coordinator is responsible to review all referrals coming into the agency. Developing and maintaining excellent relationships with our referral sources will also be an important component of this position. We are looking for someone who is organized, highly motivated, personable and aware of the customer experience. Essential Functions: Evaluate appropriateness of clients referred for home care services Coordinates all intake calls and screens for appropriateness Consults with Practitioners, case management, discharge planners and other health care providers to establish a coordinated plan of care Coordinates care with other services; ex.infusion companies Distribute referrals to appropriate clinicians that include intake data, essential background information, hospital course and plan of care Works independently, meets time deadlines, manages multiple tasks, while using discretion and independent judgement Works closely with all referral sources to establish and maintain great communication |
