Overview Home Healthcare, Hospice & Community Services (HCS) seeks a Transitions of Care Nurse for its Hospice at HCS program. This position is responsible for maintaining collaborative relationships between HCS and all referral sources to assure the patients safe transition of care from facility to home. Central to this effort is the development and fostering of excellent relationships with referral sources and the community, focusing on patient-centered activities. The successful Transitions of Care Nurse works closely with referral sources to streamline processes to ensure thorough and timely referrals and admissions. The Transitions of Care Nurse will also provide informational presentations for patients and their families in various settings (hospitals, long-term care facilities, and/or community); meet with the Palliative care team as needed regarding patients that would benefit from hospice services; perform admissions in the home following discharge from hospitals or long-term care facilities; and potentially participate in an on-call rotation to assist patients in symptom management, or to process after hours admissions and/or death pronouncements. ​
Qualifications Experience and education guidelines: Graduate of a National League for Nursing approved program, a Bachelor of Science degree in Nursing preferred; A minimum of three (3) years of satisfactory experience as a community health nurse in a licensed and/or Medicare-certified Home Health or Hospice program; Acute hospital and/or nurse liaison experience preferred; or A combination of education and experience that is determined to be equivalent to the above listed education and experience requirements; Possession of a current State of New Hampshire license to practice as a Registered Professional Nurse; Within three (3) months of hire, the employee must obtain a State of Vermont license to practice as a Registered Professional Nurse; Current cardiopulmonary resuscitation (CPR) certification; Valid driver’s license with access to an automobile during working hours that meets the State of New Hampshire’s registration regulations and auto liability insurance; and Must be able and willing to learn to utilize a portable electronic field device as well as manage electronic patient health information (ePHI) remote access in accordance with HCS policies and procedures.
